Disclosure Statement Required for All Association Volunteers

As part of an effort to help insure a safe and secure playing environment for the children of ZHAA, the association is participating in a SafeKids safety program requiring volunteers to complete a short background disclosure statement.

All adults participating in league or team activities directly involving children must complete this form. Individuals not completing the form will not be permitted to be on the field or in the dugout during league activities.

The purpose of this program is to...

• help prevent physical, mental, and verbal abuse of any of the participants, coaches, managers, employees, volunteers involved in ZHAA-sponsored activities,
• provide a safe environment and to prevent any inappropriate behavior including child abuse and sexual misconduct,
• make every reasonable effort to exclude any adult with a legally documented history of child abuse/molestation or any other conviction or record that would bring unnecessary risk to the health and safety of the participants of this organization,
• take appropriate action on all allegations of child abuse and/or sexual misconduct. All allegations will be reported immediately to the authorities for investigation and ZHAA will cooperate fully with any such investigation, and
• encourage all parents and guardians to participate in league-sponsored activities.
